Where to get a medical record number in Huntsville?

If an additional card is needed or an individual would like to enroll as a patient portal user, then a personal medical record number may be obtained by visiting the Health Information Management Department at Huntsville Memorial Hospital (936) 291-4270.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Crestwood Medical Center will send a text and / or email to the email address and / or cell phone number you provided upon registration to the hospital. Follow the instructions in that message to create your account.

To upload your insurance: Log into your patient portal. Click the third tab labeled "Insurance". Click the blue tab labeled "Add New". Fill in the required fields: Insurance Company. Policy Number. Group Number. Scroll down and click the blue tab labeled "A dd".

Do you have to activate your SHSU email?

You must activate your SHSU email before you can log in. SHSU requires to submit all medical information through our Patient Portal. Our Patient Portal allows you to upload documents from your smartphone.
