Where can I get my medical records online?

Go to MyHSHS MyHSHS Allows You To: Retrieve updated health information electronically anywhere, anytime via personal computer, laptop, mobile device or smart phone. How do I find my health records?

To do this, log in to your own account and use the proxy access request form found under the messaging tab.
