The HealtheLife Portal offers Huntsville Hospital Health System patients a convenient and secure website to help manage your health. It gives you access to a summary of the information in your Electronic Medical Record (EMR), the ability to view test and lab results, and an easy way to communicate with your health care providers.

The new name, Humboldt Park Health, signifies a renewal of its commitment to the Humboldt Park community, its patients, and their families. Placing the new tagline, Advancing Health Equity, within the new brand identify emphasizes the importance of this objective, keeping it at the forefront of its mission and purpose as an organization.
The brand refresh stakeholder team followed a multi-step process to define the new brand elements, including iconography, color, and typeface. Core elements of personality, design, and application were explored. These became the Basis of Design for the brand. The design team studied more than 1,900 branding design options, which were pared down and evaluated by the stakeholder team. The primary options were refined with input from stakeholders, who then evaluated them using a Choosing by Advantage selection process to evaluate options on quantifiable metrics.
The new name honors this diversity of ethnicities, races, cultures, languages, religions, family structures, and experiences.
A diverse team of stakeholders from across Norwegian American Hospital were deeply engaged into the brand refresh process. Members of the Executive and Leadership Teams and Board of Directors engaged in workshops to create the new brand identity. This team raised insights about the current context of Norwegian American Hospital, defined success for this initiative, laid the foundation of brand design considerations, and engaged into a rigorous process to select the preferred brand options.

Inspired from the form of a butterfly, symbolizing rebirth, the informal “H” letterform, can signify Humboldt Park Health, Hospital, or Health. The softened edges of the icon decrease its formality, reflecting our approachable and community-grounded focus on health and wellness.

This requires removing obstacles to health such as poverty, systemic racism, discrimination, and their consequences, including powerlessness and lack of access to good jobs with fair pay, healthy food, first-rate education and housing, safe environments, and affordable health care.
Dominant green reflects emotions of freshness and renewal, while blue is associated with trust and resilience. The dominant color is presented in the typeface “Health” to create a more cohesive brand and to accentuate the importance of “health.”

When it’s time for your appointment open the HHHN app (or log in from your computer) then tap or click on “Start Video Call”. Wait until your provider starts – don’t navigate off this screen. You’ll be able to see and talk with your provider (and nurse). When your visit is over, close the app or browser page.

Any active patient may be eligible to register for and use the Patient Portal. If you are authorized, a family access account can be created that will allow you to access selected family members’ health information.
Hawaii Pacific Health is following the state’s COVID-19 Vaccination Plan and federal government mandates for distribution of the vaccine to priority groups based on their risk of exposure. We started with Phase 1a of the plan, which includes health care workers and residents of long-term care facilities. We are currently vaccinating Oahu residents who are 5 and older. For appointments at our Kapiolani Medical Center for Women & Children COVID-19 Vaccine Clinic and Straub Medical Center COVID-19 Vaccine Clinic, minors ages 5 to 17 years old must be accompanied to their appointment by a parent or guardian. For appointments at our mobile vaccine clinic locations on Oahu, parents or guardians of students receiving their vaccine at a Department of Education site, ages 5-17, who will not be accompanying students for their scheduled appointment must complete the Department of Education vaccination consent form available on the HPH scheduling webpage and have their child bring it with them to their appointment.

If you are a Hawaii resident 12 years of age and older, please self-schedule below for Pier 2. Minors ages 12 to 17 years old must be accompanied to their appointment by a parent or guardian. Appointments are encouraged, walk-ins are welcome.
Before you leave, you will be asked to schedule your follow-up appointment for your second dose.
We encourage patients to self-park in the Kapi’olani parking garage. Parking in the garage is FREE for those being vaccinated and validation will be provided at the clinic.
Please note that everyone entering the facility must wear a mask and practice appropriate physical distancing.

This year we are looking forward to welcoming a new veteran, Sgt. Johnny Florez into the High Performance Heroes Racing Program. Johnny lives in Florida and will have to travel to every race up north. HPH is planning on entering Johnny into competition licensing school during the Jefferson 500.
High Performance Heroes traveled to Tampa, Fl to pick up the HPH #14 1962 Austin Healey Sprite from John Vann, who had the car for approximately two years. In Tampa, we had dinner with Johnny Florez at Mission BBQ, also had the great pleasure of meeting his wife, Maria.
