You can preregister for surgery, a test or procedure if it has already been scheduled or if admission to the hospital is planned. Epic is a worldwide leader in electronic medical records, and they have a state-of-the-art Patient Portal known as MyChart. This transition will take place from Oct. 26th to Dec. 7th, 2020.

If your insurance policy requires pre-certification or a second opinion for treatment, you need to provide appropriate authorization numbers and documentation before care is given. Also, if your plan requires a referral form, please bring it with you.
This letter will include your unique and secure Personal Identification Number (PIN) and will guide you through your new account registration process. Your PIN will only be used one time during the Identification Verification process of creating your new account. When creating your new Portal account, please make note of your Username and Password.
