You can preregister for surgery, a test or procedure if it has already been scheduled or if admission to the hospital is planned. Over the next several weeks, the UAMS Family Medical Centers will begin using a new electronic medical records system by Epic Systems Corporation. Epic is a worldwide leader in electronic medical records, and they have a state-of-the-art Patient Portal known as MyChart. This transition will take place from Oct. 26th to Dec. 7th, 2020.

The UAMS Medical Center Registration area is located on the first floor of Ward Tower. After you arrive, a UAMS Registration representative will meet with you and your family to finalize your admission and get the information necessary for your hospital stay.
If your insurance policy requires pre-certification or a second opinion for treatment, you need to provide appropriate authorization numbers and documentation before care is given. Also, if your plan requires a referral form, please bring it with you.
This letter will include your unique and secure Personal Identification Number (PIN) and will guide you through your new account registration process. Your PIN will only be used one time during the Identification Verification process of creating your new account. When creating your new Portal account, please make note of your Username and Password. You will need these logon credentials to access our Patient Portal services after registration.
