Adding Child/Spouse to Your Patient Portal • You will need to receive an email from Madison Regional Health System. This will be obtained at your next appointment or calling 605-256-6551 and asking to register for the patient portal. • Be sure to check your Spam or Junk folder if you do not see the email.
You may add more than one child to your patient portal account. A consent form is needed for each child. There are two options for enrolling an additional child: Enroll online (recommended). Enroll through physical paperwork. To put all children on one account: Use the same email address for all consent forms.
