Among them, the most widely used in hospital-wide designs is the Hospital Survey on Patient Safety Culture (HSOPS), set up in 2004 by the US Agency for Healthcare Research and Quality (AHRQ) [ 1 ]. The HSOPS is a self-administered questionnaire including 42 items used to calculate composite scores for 12 dimensions of safety culture.
While HSOPS score differences undoubtedly reflected true variations in patient safety culture, the use of different aggregation rules for the composite scores could bias the comparisons.
The Institute of Medicine (IOM)11states that a culture of safety in health care requires three elements: A belief that although health care processes are high risk, they can be designed to prevent failure. A commitment at the organizational level to detect and learn from errors.
