How do patients use a patient portal?

The Patient Portal is a secure online website that gives our patients convenient 24-hour access to personal health information with an Internet connection. It is also an easy way to communicate with your Horizon Health care team.

What are the benefits of patient portal?

How to reset password on patient portal?

Sign in to the Patient Portal, go to Security Settings, and reset your password. Click Forgot your password on the sign-in page and enter your email address to request a password reset email. Contact your provider's office and request a password reset email.

Where is medical information stored?

To prohibit unauthorized access, all medical information is stored behind our firewall in our electronic medical record system. You should always make sure that the email address on file for your account is accurate, as notifications from the portal are sent to the email address on file.

What happens if your keyboard is idle?

Alternatively, if your keyboard remains idle for 10 minutes or more, you will receive a pop-up window asking if you are still actively using the portal. If you do not click the OK button, you will be signed out automatically. Any information you have typed and not saved or sent will be lost.

Can you make your test results available on the patient portal?

It is at your provider's discretion to make test results available. Your provider must authorize the release of your test results in order for them to post to your Patient Portal account. Only test results which are considered appropriate for release will be accessible through the Patient Portal.

How long does a patient portal code last?

No, you will use this code only once to log in to the patient portal for the first time. (The code expires after you have used it or after 30 days). When you log in to the portal for the first time, you will need to create your own unique Patient Portal ID and password.

How long does it take for a patient to get test results?

Test results are released to your patient portal account after your physician has reviewed them. This is generally in 10-14 days. Your provider determines which types of test results can be accessed through the Patient Portal. Test results of a very sensitive nature are not released to the Portal.
