What is the MIHP? MIHP is a Home Visiting program for pregnant women and infants with Medicaid insurance. MIHP supports healthy pregnancies and healthy babies.

Emergency Information

One of our board-certified physicians is always on call for emergency and labor and delivery needs. The on-call physician provides for your care after office hours and on the weekends. For after hours emergency care call 616.377.4800.

In order to make your appointment go as quickly and smoothly as possible, please print and complete the following forms and bring them to your appointment along with your insurance card, copay, a photo ID and medical records if you have them. If you’re a new patient, please arrive 15 minutes before your scheduled appointment time.
