What is Heartland Community Health Center?

Heartland Community Health Center serves Lawrence, KS and surrounding areas with primary care, dental care, and mental and behavioral health care . We are dedicated to practicing holistic, integrated health care – a health model that focuses on physical health as well as emotional, and social health. With the integration of mental and behavioral health services, Heartland acknowledges that we are more than just physical bodies.

Is Heartland a PCMH?

Heartland is a Federally Qualified Health Center and a recognized Patient-Centered Medical Home (PCMH) that offers its patients accessible, coordinated and compassionate care to ensure that all needs are met.

To Get Started

Patients need a personal identification number (PIN). Contact the front desk staff at your health center for your PIN. Then go to www.hhcportal.org to register. You can also use the patient login button on this website.

Community Health Centers

These locations are open to the entire community. We accept Medicaid, Medicare and private insurance. If you are uninsured, we have affordable patient visit fees based on sliding scale.

School-based Health Centers

Our health centers include seven school-based health centers at local elementary and high schools. Appointments for a health care visit are limited for students who attend these schools.

Integrated Care Locations

Heartland Health Centers has primary health care centers at three sites located within agencies providing mental health and substance abuse services. Appointments are available for the clients of these organizations.
