What can patients do in the patient portal?

Patients can access their health records online, anytime, anywhere, and on any device. Through the Patient Portal, patients can access lab results, visit summaries, review billing statements, view upcoming appointments, request medication refills, send direct messages to their Provider, and much more.

What is the MyChart patient portal?

How do I Share my patient information with other providers?

Share your information with caregivers and other providers You can create and access your account via the Plateau Medical Center website or by downloading the MyHealthHome – MphRx mobile app available in the Apple App Store or on Google Play.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Plateau Medical Center will send a text and / or email to the email address and / or cell phone number you provided upon registration to the hospital. Follow the instructions in that message to create your account.
