How to obtain medical records?

Medical records will only be released when the requester provides proper identification. Photo identification is required. If you are requesting records for someone other than yourself, the patient must have completed the authorization form giving you authority to obtain copies. In certain circumstances, you may be asked to provide supporting legal documentation such as: 1 Power of Attorney 2 Advance Health Care Directive 3 Living Will 4 Executor of the Estate 5 Court order/Conservatorship appointed by the court

Benefits of viewing your own medical records. Reviewing your medical history can help you make important choices regarding future healthcare. If you move frequently, or often visit numerous doctors, providing them with easy access to your medical records can help facilitate their treatments and advice.

What is the phone number for the medical records department?

Call us if you have any questions. The Medical Records Department can be reached at (719) 530-2285, Monday – Friday, between 8:00 a.m. and 5:00 p.m.

Can you get results from HRRMC?

If you or someone in your household has been tested at HRRMC for COVID-19, do not come to the hospital to obtain your results. You will be contacted with your results. The department in which you were tested will contact you with the results (whether that is the Emergency Department or your provider’s office).
