Care Transition Support

Have you ever been discharged from the hospital and did not know what you were supposed to do next? Were you confused by all the papers you were given at discharge? HealthChoice offers care transition support designed to help you after your discharge from the hospital or emergency room (ER).

Chronic Kidney Disease and Dialysis Transition

Learning that you have kidney disease and may need dialysis can cause fear and uncertainty for the future. Maintaining a healthy and productive lifestyle is important. We provide care coordination based on specific needs related to Chronic Kidney Disease (CKD), End State Renal Disease (ESRD) and Dialysis.

Complex Care Management

Through our Care Transition Support program, we identify members who may have a medically complex illness or multiple chronic conditions. Juggling doctor visits, hospital and ER visits, or multiple medications, can be very overwhelming. Our Complex Care Manager can help you take control and navigate confusing healthcare systems.

HealthChoice as an ACO

HealthChoice is also an Accountable Care Organization (ACO). Our network of doctors, hospitals, dialysis centers and other healthcare providers work together with our Nurse Care Managers and Medical Assistants to coordinate your medical care.
