You can use the Patient Portal to request that UHS send your medical records to you, another medical provider/facility/institution, or insurance company. Login to your account and click the "Forms" tab. Under Section A: UHS Forms, click "Release of Information UHS.". Complete and submit the Release of Information form.
Patient Portal accounts must contain a unique email address. Minor children typically do not have their own email address. Due to HIPAA regulations and patient confidentiality policies, these accounts cannot be created at this time.
All enrolled students are required to be fully vaccinated for COVID-19 to live, learn or visit any UMass Lowell campus or property. According to the CDC, fully vaccinated is two weeks after the second dose of a two-dose series or single-dose vaccine.
All newly enrolled students should complete the health forms online and upload supporting medical documentation through the Health Services Patient Portal link above.
All health records are required by Aug. 1 for fall enrollment and Jan. 10 for spring enrollment.
Health Services Patient Portal: An online patient portal is available to help your student manage their health care needs. Your student can register for a portal account using their UMass Lowell email account, and can use the portal to upload insurance and immunization information, view and cancel upcoming appointments, complete mandatory, university required health forms, and access health education information.
