How do I get to my Patient Portal?

Patient care pathways begin once medical imaging is accessible to care givers and a diagnosis is provided. How long does it take to get a health nucleus report?

Personalized Health Reports – In approximately 8 weeks after your Health Nucleus visit, your complete Health Nucleus report session will be scheduled. At this time, our team of multidisciplinary specialists will have reviewed and assembled the data into a summary report where we consolidate your genomic findings with your phenotype (physical) data.

Why is the nucleus important?

Health Nucleus can be an invaluable resource for staying a step ahead of diseases with a strong genetic component. With whole genome analysis, we can identify genetic variants that may increase or decrease your risk for different conditions and diseases. Combining these findings with the whole-body MRI scan can provide insights on your current health and uncover early signals of these diseases.

What is pre-visit consent?

Pre-Visit Consent – We want you to be prepared for your visit, which includes understanding the clinical and research aspects of our program. To ensure that you understand and meet the eligibility requirements to participate in our research studies, as well as to inform you of what is included in the testing on day of visit and results delivery, we provide an informational call prior to your Health Nucleus visit. We will provide you with consent documents that outline the research studies for your review prior to you coming on site. You are also given the opportunity to speak with a member of our clinical team to ask any specific health questions you may have.

What is a whole body scan?

A whole-body scan that can detect some solid organ tumors. This protocol uses advanced diffusion imaging based methods to differentiate high-grade cancerous tumors from benign or low-grade lesions.

What is personalized knowledge?

Personalized knowledge about your individual health risk factors based on your genomic and phenotypic (physical) information, not a national average. Early identification of diseases that may not have otherwise been discovered until the development of symptoms.

Is the Health Nucleus platform diagnostic?

The Health Nucleus platform is for screening and assessing one’s risk for different diseases, as well as early signals of disease risk. This is not a diagnostic disease testing platform.

Can a physician review your results?

Our aim is to empower you and your primary-care physician to integrate your findings into your ongoing health plan. With your consent, we can include your personal physician in the results sessions, or our clinicians can review your results directly with your doctor.

How is health information stored?

Your health and financial data are stored using secure encryption and security technology to protect your information . Patient information is always encrypted, firewall-protected and is under surveillance 24/7.

How long does it take to connect Nuvance Health to my health app?

Can providers update results?

Providers will update results and answer questions during their regular daily visit. If patients have questions about information in the portal, they can ask a member of the care. Can you see results of a genetic test before they are available?

Except for genetic testing, patients will receive test results as soon as they are available. It is possible patients will see results before their provider has had an opportunity to review them. Nuvance Health will contact patients about abnormal test results as quickly as possible. 8.

What to do if you have issues logging into your patient portal?

If you’re having issues logging into your patient portal, please contact your medical provider’s office for support.

Can you use a portal locator on a patient portal?

Yes. Using the portal locator doesn’t affect your ability to use other URLs to log in to your patient portal.
