Obtain a “Patient Portal Welcome Letter” from Peninsula Kidney Associates.

In your portal, you will find

Your health information including labs, health history, family history, etc.

Request an Appointment

Radiology Associates offers appointment requests online for most exams.#N#Please Click on “Request An Appointment” button below then#N#select the exam you are requesting and fill out the form.#N#Our Patient Services Coordinator will contact you by phone with a day and time for your appointment.

To ensure accurate scheduling and excellent patient care, we require a copy of your order form you received from your doctor. You may take a picture via your smartphone and email it to orders@ra-slo.com
