PHMG launched the patient portal in early 2010. As a first step, the physician champion piloted the portal for about 6 months before it was implemented in one clinic at a time. According to the physician champion, implementation was “easier than expected because everyone was already comfortable with eClinicalWorks, ...
One major challenge with the portal is the multiple step registration process . Patients provide their e‐mail address at the front desk and are given a password to register from home. Some patients fail to complete the registration process after leaving the clinic. Remembering and managing passwords and managing family accounts are also challenging for patients. For example, a parent may log in for one child and then ask questions about a second child. For providers and staff, a challenge is that there is no way to know whether a Web‐enabled patient actually uses the portal and there are no read receipts to confirm that patients have read a message.
PHMG had a strategy of ensuring that patients hear about the portal from multiple sources during each clinical visit. To execute this strategy, PHMG used several methods of communication, including:
They found that it is particularly persuasive when providers encourage patients to use the portal because patients trust providers and value their opinions. One provider says he reinforces a patient’s use of the portal by closing all messages with “Thanks for using the portal.”.
PHMG is currently working with Healthwise® — a nonprofit organization based in Boise, Idaho, that develops health content and patient education solutions—to beta test the integration of Healthwise patient education materials into the eClinicalWorks EHR system.
Qualis has also been an important resource for information about the meaningful use rules. "We felt strongly that from a quality standpoint we could not succeed without going to electronic health records. I felt very strongly we had to invest in it because it would positively affect every patient that we encounter.".
In 2007 PHMG implemented an EHR system, eClinicalWorks, as part of a strategy to improve quality of care and facilitate coordination of care across its multiple clinic locations. In preparing for implementation, PHMG proceeded with:
Patients are also engaging with their own health using digital health tools. One-third of patients use an electronic device to monitor their health, including fitness wearables and blood pressure monitors. Forty percent of patients use a wellness app on their smartphones or tablets.
Those patients likely did not perceive the record as useful because they could not understand it. Patient portal developers and the clinicians who use them should ensure that this information is accessible, understandable, and written in lay language where applicable.
April 16, 2018 - Patient access to health data is continuously increasing, but there is still more opportunity for medical professionals to encourage better patient engagement with their own health data, according to a new ONC data brief about patient technology use.

Schedule immediate appointments for patients with complicated symptoms or conditions. Implement a policy to refer patients with basic symptoms to a physician assistant (PA) or nurse practitioner. It is not necessary for them to see a doctor or specialist. Hire part-time nurses, PAs, or providers.
The first step for any health organization/system should be to implement a method of monitoring and measuring patient access and patient workflow. This means that you should know how long on average a patient must wait before they can get an appointment at your facility/facilities.
The whole goal here is to improve patient access to your health system. Therefore, you should be evaluating each of the programs to ensure that it is having a tangible positive effect on access.
The most extreme case was in Phoenix where 35 veterans died while waiting for care (Source). Now, there were a number of problems with the VA hospitals that allowed for something like this to happen. Mainly, these hospitals were heavily understaffed and held to unrealistic standards.
There should be someone in charge of each solution so that multiple things can be accomplished within the same time frame. In addition, there should be one person (the office manager for example) that oversees the entire project to ensure that everyone and everything is in order.
Patients may be complaining that they need to see the doctor immediately. However, the receptionist or scheduler is not trained to know whether certain complaints are severe or not. These are all patient access issues, and therefore, you and your staff must come up with solutions to fix these problems.
