To expedite your first appointment, please arrive a few minutes early to complete registration forms so that we have all the necessary information to treat your child. You may also download and print the forms from this website, fill them out ahead of time, and bring them with you to the first appointment.
A pleasant, comfortable first visit builds trust and helps put the child at ease during future visits. We want your child to enjoy getting to know our doctors and staff, so we work hard to establish this bond during every appointment to our office.
Everyone over 2 years of age MUST wear a mask to enter the office and throughout the visit. Masks with exhalation valves and gaiters are not permitted for use in the office. Please wear a surgical or cloth mask that covers the nose and mouth.
We have recently seen an increase in COVID-19 activity in our area. Due to the high volume of calls related to this, we have provided links to the CDC and GA Department of Health that outline recommendations for quarantine and isolation guidelines related to COVID-19.
GPAM Statement on COVID-19 Vaccination in Children 5 years old and older:
Gwinnett Pediatrics is excited to announce we have the Pfizer vaccine for our patients 5-11 years old. Unfortunately, we no longer have the vaccine for ages 12 and up. We do recommend vaccination for children 12 and up, you can go to the health department or the local pharmacy. It is both safe and effective at preventing COVID-19.
