Deceased patients: To obtain a copy of a deceased patient’s record, you must complete, date and sign an Authorization Form for Release of Medical Records and Information . If the requester is a minor, proof of emancipation, authority and identity is required, such as: Executor of Estate or Court Order Administrator.
Minors (patients under 18 of age): The parent or legal representative must complete, sign and date the Authorization Form for Release of Medical Records and Information, unless the minor is emancipated. A legal guardian may be asked to provide proof of guardianship.

When the patient is a minor being treated for substance abuse, the minor and the parent both may be asked to sign the Authorization Form for Release of Medical Records and Information, regardless who consented for treatment. Patients 18 and older: Under most circumstances, patients over 18 must sign for themselves.
Note: If minor consented for their outpatient treatment for pregnancy, sexually transmitted disease or behavioral/mental health without parental consent, the minor may sign the Authorization Form for Release of Medical Records and Information.

Gwinnett Pulmonary offers a variety of services related to helping any lung diseases or issues. The first service offered is the Pulmonary Function test. Pulmonary function tests (PFTs) measure how well your lungs take in and exhale air, and how efficiently they transfer oxygen into the blood. Spirometry measures how well the lungs exhale (breathe out). Lung volume measures how well the lungs inhale (breathe in). Testing the diffusion capacity of carbon monoxide (DLCO) shows how efficiently the lungs transfer oxygen from the air into the bloodstream.
A pulse oximeter is a medical device that indirectly measures the oxygen saturation of a patient’s blood (as opposed to measuring oxygen saturation directly through a blood sample) and changes in blood volume in the skin, producing a photoplethysmograph.
Lastly, Gwinnett Pulmonary offers Cardiopulmonary Exercise Testing. This is a non-invasive and objective method of evaluating both cardiac and pulmonary function. It allows a very accurate assessment of possible cardiac failure, in a quantified fashion.
