Without limiting the forgoing, account deactivation typically will result from: 1) Failure to abide by these Terms and Conditions; or 2) Inappropriate use of messaging (e.g., messaging that is excessive, unnecessary or contains inappropriate content). We will attempt to notify you if your account is deactivated.
To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.
