Eligible employees may enroll in a GuideStone Personal health plan without providing an Evidence of Good Health Application if they experience one or more of the special enrollment events listed below. Employees must notify GuideStone within 60 days of the date they experience the event.
To qualify for GuideStone Group Plans: Employers with two to four covered employees must contribute 100% of the employee-only cost of medical coverage. Employers with five or more covered employees must contribute a minimum of 50% of the employee-only cost of medical coverage.
