How do I enroll in the patient portal?

To enroll, please download, read, sign and return the patient portal agreement. Once we receive your signed agreement, we will issue your secure user name and temporary password. After your initial login, you will create your own personal password.

Current functionality of our Patient Portal: communicating with our practice securely and efficiently requesting an appointment and viewing your current appointments requesting a prescription from a pre-populated list of currently refillable prescriptions managing and updating your personal information
