Adult Foster Care

We provide our clients with help for all activities of daily living; this includes medication assistance, injections, and other routine care; assistance with personal care, including bathing, dressing, grooming, hygiene, and help managing challenges created by confusion. We have an RN available to delegate skilled nursing tasks.

Behavioral Health

The Confederated Tribes of Grand Ronde Behavioral Health Program (CTGR-BHP) is a fully integrated outpatient mental health, alcohol and drug assessment and treatment program licensed by the State of Oregon.

Dental

The Dental Department strives to educate and improve the overall health /dental health of our patients through comprehensive, quality dental services.

Medical

Primary care services are provided by a skilled team of licensed practitioners, including family practice physicians, physician assistants, nurse practitioners, and nurses. We also offer walk-in clinic services to provide care for minor injury or illness.

Optometry

We provide routine eye examinations including screening for glaucoma and complications from diabetes and other diseases. Refractive care includes dispensing glasses and contact lenses. After we’ve made sure your eyes are healthy (or are receiving proper treatment), we’ll discuss any needs you have for glasses or contact lenses.

The GRH Behavioral Health Clinic provides a broad range of services including assessment, education, brief intervention and longer-term therapy. Our goal is to help our patients not only manage mental health disorders but to skill build for general overall health.
