If you need to request your medical records be sent to an outside clinic you need to complete a release form. You can complete your Authorization Form for Release of Information online, in-person, by mail, or fax

Release of Information for Minors

If you are completing the Authorization to Request Medical Records for your child the parent/legal guardian MUST sign the document and state their relationship to the patient.

Self-Pay Information

Don't have insurance? We offer self-pay services. Sometimes you are not eligible for insurance through your employer or are between jobs and do not have any insurance but still need to be seen. This is why we offer self-pay services at our clinic.
