Accessing Medical Records
Your hospital medical records can be accessed by logging into the MyGraham Patient Portal, our confidential website that allows you to access information within your medical record. Patients may review instructions, test results, and information regarding your most recent hospital stay including your discharge medication list.
Requesting Medical Records
In order to protect your privacy, we require written authorization before medical records are released. To obtain copies of medical records, patients will need to complete an Authorization for Release of Information form.
Please allow up to ten business days to process your request. Records will be mailed to the address placed on the Authorization for Release of Information form. Records may also be picked up if prior arrangements are made with the Health Information Management department.
