What is MyChart Grace clinic?

Beginning in early December, your medical records for Grace Clinic® and Covenant Health Grace Surgical hospital will be accessible in our new patient portal, MyChart, offering you more convenience and giving your entire care team a more complete picture of your health.

Where can I Find my medical records for Grace clinic?

Thank you for choosing Grace Medical Center as your healthcare provider. Our Patient Financial Services Department is available to assist patients who do not carry medical insurance (uninsured) or face significant co-payment, coinsurance and/or deductible charges, which may be challenging to manage due to personal hardship or financial distress.

What is the MyChart patient portal?

I am a Grace Clinic patient with an existing MyChart account. Do I have to sign up again?

Good news, since you’ve already signed for MyChart with Grace, you can continue to use your existing MyChart account to access your Covenant Health information too.

When can I start using all of the features online or in the app?

You can check in online for your next appointment up to four days before your visit. Other features will become available on March 5, 2022 including: appointment scheduling, messaging, test and lab results, prescription information, billing, and more.

What information will be available online or in the app?

Most of your medical history with Covenant Health will transfer to MyChart. Please note some information may not be fully available in your account until the end of the year.

Why are you making these changes?

These upgrades are part of our commitment to better serve you. Our new system will make it easier to access your medical records and get care when you need it.

Can I check in online before my appointment?

You’ll be able to use our new online check-in up to four days prior to your appointment, when convenient for you. To get check-in reminders, sign up for MyChart to review your profile and contact info.

What else does this mean for me?

Your appointment may take longer while we adjust to our new system, and you may need to sign documents again.

How will this affect my care?

You’ll continue to get world-class care at all our locations. This upgrade means your records will be in one place for all Covenant Health and Grace providers to view.

My Health Rec is a great way to view, download and transmit your up-to-date health information, all from the convenience and privacy of your own home, or anywhere Internet access is available — 24 hours a day, seven days a week.

How to Get Started

Get started today by enrolling in My Health Rec at your next inpatient visit. You can also set up your own patient portal account by using our self-enrollment tool. My Health Rec is available for all patients who have registered and had services provided to them from the facility.

Accessing Your Account

  • If you forget your password, select the “forgot password” link on the login page. Once you’ve answered a few questions, you’ll be able to create a new password and access your account. After 8 failed login attempts, your account will be automatically locked. If your account is locked, you’ll need to bring an ID to your GraceMed clinic to verify your account.
