All newly admitted or newly re-admitted students are required to submit the Mason Immunization Record Form by the deadline. Students must complete Parts 1 – 3. A healthcare professional must fill out (transcribe) the remaining sections for students born after December 31, 1956.
Proof of vaccination or immunity (through blood work) must be supported by documentation. A healthcare professional will complete and sign the Immunization Record Form based on historical documentation or administration of shots. Copies of any lab reports for titers must be submitted with the Immunization Record Form.

Student Health Services is responsible for collecting and maintaining students’ immunization records. Required Immunizations. Tuberculosis: Tuberculosis (TB) screening is required for all students. If TB testing is needed it must be completed within 3 months of the first day of classes for the semester.
Once records are submitted and a student has received a secure message in the patient portal indicating a missing immunization, students can call the Immunization Office at 703-993-2135 to schedule an appointment for needed immunizations.
Minor Consent. A parent/legal representative signature is required for a minor (a student under age 18 at the time classes begin). A minor consent must be on file at Student Health Services to receive most health services. This minor consent is on the immunization record form.

For telehealth, you will need a private location, device capable of audio and video transmission, like a smartphone or computer, stable internet connection, and if needed, headphones. Good lighting is also very helpful for video telehealth.
Telehealth is the use of audio and video technology to offer services to patients who are not in the same physical location as their provider. Current Mason students in Virginia are eligible for telehealth appointments. Healthcare providers are bound by the laws of their state and cannot provide care outside of that scope of practice.
