Glacial Ridge Health System implemented a new patient portal in September 2020, upgrading from InteliChart® to MyCareCornerTM. A transition was needed as the prior portal is no longer supported.

How do I create a new account?

If you are a new patient at GRHS, have never used our patient portal, or missed the timeline to transfer your prior patient portal to the new one, the most convenient option is to create a new account by clicking on the button above.

Community Health Center of the North Country's Patient Portal offers patients easy and private access to their own medical information on their personal computer or mobile device, so patients can view their own health record whenever and wherever they have access to the internet.

How do I get access?

During your health center visit, you may ask any member of our Registration/Reception team to register you for easy access to our Patient Portal. You may also call (315) 379-8100.#N#After your account has been created, visit our website and click on the "Patient Portal" button.

How long does it take for a pharmacy to process a refill request?

Please remember to allow 48-72 hours for the practice to process your refill request. You can also notify the practice of medications you are no longer taking or of any new medications. You can use the portal to notify the practice if you change your pharmacy.

The Patient Portal website is compatible for use on various devices. In addition to a desktop site, a mobile site with a layout optimized for a smaller screen size is also available. The menus and screens are programmed to be easy to use on a smartphone and an icon can be placed on the Home screen for quick access.
