History

Georgia Mountains Health is a Federally Qualified Health Center and a FTCA deemed provider of medical, dental, and behavioral health services. Founded in 1984, We currently have offices in Blue Ridge, Chatsworth, Dahlonega, and Ellijay.

Patient Centered Medical Home

The National Association for Quality Care recognized all Georgia Mountains Health medical sites as a Patient Centered Medical Home. This recognition was earned based on meeting high standards in the delivery of care to patients. We are the only practice in our service area receiving this national designation.

Insurance

Veterans are welcome at our office! We accept Tricare and VA Health Care Benefits. Please call the office for more details. We accept Medicare, Medicaid, CareSource, WellCare, Amerigroup, Peach State, Blue Cross, United Health and many other insurance plans.

Discount Policy

All primary health services are available on a sliding fee scale based on family income. Patients who qualify will be entitled to a discount on medical, dental, and behavioral health care. The office receptionist has details for those interested.

Discounted Medications

We participate in the 340B Drug Assistance Program. Under this program patients without insurance may qualify for discounted medications from one of the 12 local pharmacies in our network.

Can you add a dependent to a patient portal?

Yes. You can add family members that you manage and when you log into your Patient Portal. Simply click on “My account” in the top right hand corner, and then “Add a child/dependent.”.
