The Healthcare to the Homeless Program at Generations, provides funding for primary healthcare and linkages to a health care home through ongoing case management and outreach provided at our health center sites and at locations in the community for people whose primary barrier to care is lack of permanent housing. Our Care Coordinators and Community Health Workers establish scheduled hours at local shelters, soup kitchens, housing projects and agencies to provide immediate access to health services for people sleeping on the street, in shelters, residing in substandard housing or transitional programs, or doubled up with others temporarily. This Program is funded by the Federal Bureau of Primary Care.
In addition, Care Coordinators are knowledgeable about other health programs and funds for those without health coverage who may not be eligible for HUSKY Health and will link them to resources for medication assistance, preventive health screening programs or other health services they may need.
The Eastern CT Medical Home Initiative for Children & Youth with Special Health Care Needs at Generations provides care coordination services to children and youth up to age 21 with special health care needs and their families/caregivers. Children & Youth with Special Health Care Needs have additional health and related service needs beyond that required for children in general and through this program families/caregivers can receive information and supports.
Uninsured and underinsured. Children and youth with special healthcare needs (autism, asthma, diabetes, development issues etc) People living in transitional situations or at risk for doing so. (homeless, shelters, programs) Agricultural farm workers and their families.
Children up to age 19 may qualify regardless of family income level. The Care Coordinators will work with each individual and family to determine which program is a good fit and help to complete the application.
Generations employs bilingual, culturally competent staff. Staff work closely with clinicians at Generations and alongside community partners to make sure that individuals and families seeking healthcare have an advocate to assist with navigating the system and obtaining the care they want or need.
