How do I sign up for the patientportal?

Please send an email to: PatientPortal@chcrr.org How do I sign up? Be or become a patient at any one of our practices. You will need a portal activation code. (To obtain your portal activation code, call our Centralized Medical Records department at 802-671-5800, call your provider’s office, or stop by your provider’s office.)

You will need to provide Patient Registration with your personal, non-shared email address by visiting our campuses or contacting Athol Hospital Patient Registration 978-249-1355 or Heywood Hospital Patient Registration 978-630-6531.

What devices can I use the patient portal website on?

The Patient Portal website is compatible for use on various devices. In addition to a desktop site, a mobile site with a layout optimized for a smaller screen size is also available. The menus and screens are programmed to be easy to use on a smartphone and an icon can be placed on the Home screen for quick access.

You can sign on to our Patient Portal. You can also download myCHC, our Patient Portal App to connect with your provider and access your medical record.

What is Patient Portal?

The Patient Portal will help you better manage your health care, message your provider, renew a prescription, request an appointment, pay your bill or update your health record.

How to get portal activation code?

How to request an appointment with a provider?

You can also request an appointment by completing the form. Just choose the provider you would like to see, the reason for the appointment, and the days and times that work best for you and click submit. Someone from the provider’s office will contact you with an appointment.

Can you update your medical history on the portal?

You will have the ability in the portal to update different sections of your history and send information to your provider to have added to your medical chart.
