Our geriatric physicians serve as co-primary care physicians or geriatric consultants who provide health assessment and management for older adult patients in concert with their primary care providers.

A written request can be made to Florida State University Registrar’s Office with a copy of supporting documentation of the legal name change (i.e., marriage license, driver’s license etc.). Once the student is notified of the name change, they will need to contact University Health Services to update their Electronic Medical Record.

Medical records contain complete records of each visit to the health center, health history forms, documentation of any shots, and copies of reports from any diagnostic studies (lab, x-rays, EKGs etc.). Medical records of patients 18 years of age or older are kept strictly confidential.
