What services does freestone Clinic offer?

Because we are affiliated with Freestone Medical Center, our Clinic can refer you to local medical services for a wide of diagnostic procedures such as x-rays, ultrasounds, CT scans, bloodwork and more. We are an in-network provider for Medicare, Medicaid and most private insurance plans.

How do I sign up for the medical records portal?

The staff members in the Medical Records department will be able to walk you through the process to sign up to the portal. Online self-enrollment is available if you are 18 and over. You will need your medical record number in order to successfully register online. Online self-enrollment is currently not available for caregivers or guardians.

Is my health information safe in a patient portal?

Your Information is Private, Secure, and Protected. Patient portals have privacy and security safeguards in place to protect your health information. To make sure that your private health information is safe from unauthorized access, patient portals are hosted on a secure connection and accessed via an encrypted, password-protected logon

Freestone's History

Freestone Medical Center opened its doors in 1947 as Freestone County Cooperative Hospital. The original facility underwent a number of expansions and renovations over its first 25 years. Then in 1975, a new facility was constructed and opened under the name Fairfield Memorial Hospital.

If you do not have a scheduled appointment and wish to sign up for My FMC Portal, you need to go to the FMC Medical Records Department, which is located on the first floor at 401 N. Ewing St. in Lancaster, Ohio. The hours are 7 a.m.-8:30 p.m.
