About Us

Frederick Primary Care Associates specialize in Family Medicine. At our offices located in Frederick (2 locations), Ballenger Creek, Brunswick, Jefferson, Spring Ridge, Woodsboro, Walkersville, and Urbana, we are able to provide comprehensive medical care for newborn through geriatric patients.

Appointments

We offer daytime, evening and Saturday appointments. Call our access center to schedule an appointment at (240) 215-6310. Your access agent will ask specific questions to determine an appropriate appointment length. To request an appointment online, please fill out the form below:

Walk-In Appointment

We offer Walk-In Care. Walk-In locations are housed inside of our Urgent Care Locations. Upon arrival please tell the receptionist that you are a Frederick Health Medical Group Primary Care Patient since this a different service from Urgent Care.

Online Patient Portal

Communicate directly with your care team through our online patient portal. With an email address, it is quick and easy to enroll. In addition the portal offers an option to request refills of your medications and review imaging and laboratory results.

Patient Centered Medical Home (PCMH)

Frederick Health Primary Care sites employ an innovative model of primary care called Patient Centered Medical Home (PCMH) where care is organized and evidence-based. We have been recognized by the National Committee of Quality Assurance (NCQA) as a Level 3 Patient Centered Medical Home (PCMH).

Patient Forms

Health History – Provides our office with a full medical history for the patient. Authorization for Release of Medical Information – Allows patients to authorize the disclosure of their health information to a designated individual, company, agency, or facility.

Office Policies

FPCA Financial Policy – This form advises patients of their complete financial responsibility for all medical services received without regard to insurance eligibility or coverage determinations.
