How do I get Set up on the patient portal?

To get set up on the patient portal, you will receive an email following your appointment with your login and temporary password. Please use that info to login and set a password of your choosing. Please be aware that we will not have any access to your password.

New User

If you are a new user and need access to the portal, please complete the Provider Portal Access Request form and e-mail to the Provider Relations Representative, cstjohn@fvmedicine.com. All new user requests must be in writing.

Password Reset

If you need to reset your password for the portal, e-mail eznet@innovista-health.com. When sending the e-mail request, please indicate in the subject line of the e-mail "Password Reset". The following information must also be included in your e-mail: • Name • Username • Phone Number • Practice Name • Physician Name (s) Associated with Practice.
