We are happy to offer Foundation eCare, your patient portal. Foundation eCare is an online service for patients of Foundation Medical Partners. To set up your account, visit the office to obtain your PIN. Once you have your PIN, click on the Foundation eCare logo to the left to set up your account.

How do I obtain an Ecare authorization form?

Foundation eCare is disabled. You may register for your new patient portal, SolutionHealth MyChart, by following the instructions on your after visit summary or by calling 888-243-7153.
