What should I do if I have technical issues with patient portal?

Existing patients may call 561-432-5849 to schedule an appointment. Existing patients may also schedule an appointment through My Health Record, FoundCare's online patient portal. If you would like access to My Health Record, please speak with one of our front desk staff at the time of your visit to obtain your invitation to join.

Sliding Fee-Scale

Our goal as a community health center is to provide comprehensive quality health care to our patients. Every patient seen by FoundCare providers receives the same high-quality standard of care regardless of whether or not they have insurance. We offer a sliding fee scale based on family size and income for those who are not covered by insurance.

When your doctor writes you a prescription, we’ll be there to coordinate all aspects of your therapy. You can rest easy knowing we’re working with your physicians, care team and insurance companies to ensure everything is taken care of. We’ll even deliver your medications directly to your home, workplace, or wherever is most convenient.

What to do if you have issues logging into your patient portal?

If you’re having issues logging into your patient portal, please contact your medical provider’s office for support.

What do you do if you are an authorized caregiver?

If you are an authorized caregiver that needs help accessing a family member’s account, you will need to contact that family member’s medical provider for support.

Can you use a portal locator on a patient portal?

Yes. Using the portal locator doesn’t affect your ability to use other URLs to log in to your patient portal.
