How do I access my health information?

Patients who have received care at the hospital or one of the medical buildings can access their Health Information by logging in to the corresponding portal. For assistance accessing your patient portal, resetting your password or account, call (937) 547-5732.

To create an online account with My Kid's Chart, go to the Patient Portal Registration page and follow the instructions. Please complete the Patient Portal Registration Form for our office records. You can submit this form at your next visit.
