How do I set up a new MyHealthHome (Cerner) account?#N#Sign-up is simple and takes place as part of a visit to an LHP office or during a hospital stay. Once you sign up, watch for an email from noreply@iqhealth.com. Click the enrollment link to complete the process.
How can I access my child's or other family member's health information?#N#If you are interested in MyHealthHome access for your child (ren) or an adult for whom you are legally responsible and that individual is a patient of a participating provider, you will need to sign up in person by completing an Authorized Proxy User Access Request Form.
Will my portal information be secure?#N#Yes. MyHealthHome uses a secure, encrypted connection and all health data is stored on a secure server.

If paperwork was not received in the mail, new patients will need to complete the paperwork prior to seeing the doctor.
