Accessing Medical Records

Normally, Medical Records are requested in person, however, because visitor restrictions are now in place at this hospital, you can request medical records by downloading this Release of Information form, filling it out and either faxing it to 870-261-0440 or mail to: Forrest City Medical Center, HIM Department, 1601 Newcastle Road, Forrest City, AR 72335, ATTN: Christi Story.
