Through this portal, patients can access and manage their health demographics, insurance information, exam details and communicate with your provider electronically.
You can access your medical care details and interact directly with our practice online from anywhere through the patient portal. Foothill Family Clinic understands the value of communication when it comes to your healthcare and we are thrilled to offer this service to our patients.
There are two ways to receive your YourHealth Hospital Patient Portal credentials. On your next visit to one of our affiliated hospitals, registration will request an email address from you. If provided, registration will print out a form with sign-up instructions and four pieces of your medical information:
Our patient portal is a protected site where only you or individuals you designate can have access to your information. It’s easy, user friendly, and designed to help you stay informed, healthy and safe.
There you can create your profile, manage your settings and customize your password. 1. During your visit, provide your personal email address to any of our staff members. 2. You will receive a token which is used to access your on line portal.
If you have an urgent problem that occurs after hours or on the weekend, phone the office where you are seen, and you will reach our exchange. You will either be asked to stay on the line, or they will take your number and notify the on-call physician. Doctors alternate evening and weekend emergency calls, enabling us to always have a physician available.
Your medical record is strictly confidential. We will not give out information regarding your condition to your employer, relatives or friends without your written permission. The only exception to this is when disclosure is required by law.

If an upcoming appointment is eligible for eCheck-in, you can use it to take care of tasks such as checking in ahead of your appointment, updating personal, medical and insurance info, eSigning documents and answering questionnaires.
For the online portal, log into MyBCH, hover your cursor over the Profile icon in the top menu and select Personal Information. For the app, log into MyBCH click the three dots on the top right corner of the screen and select Account Settings.
A parent or guardian can use MyBCH to request proxy access to a minor’s records. Sign in to the online portal, and under Profile > Personalize > Request access to a minor’s record. These requests will be reviewed by the BCH Health Information Management (HIM) department. A patient can call HIM at 303-415-7760, then press #3, Monday-Friday, 8 a.m.-5 p.m. This is only for non-legal relationships. This proxy request is not currently available in the MyBCH app.

You can schedule a video or phone visit by calling your clinic directly , calling the BCH scheduling number at (303) 415-4015 or by requesting an appointment through your MyBCH portal.
