How do I sign up for my FMC portal?

There are two options to sign up for My FMC Portal. At your next scheduled visit you can provide your email address to a Fairfield Medical Center team member. They will be able to send you a secure email invitation to sign up for My FMC Portal.

How do I sign up for Franklin Medical Center’s Patient Portal?

Within a few hours, you will be sent an email with a link to Franklin Medical Center’s patient portal. Once you click the link you will be required to verify your identity and create an account. Alternatively, you may self-register here.

When does the invitation to join my FMC portal expire?

This invitation to join expires after 90 days. If you do not have a scheduled appointment and wish to sign up for My FMC Portal, you need to go to the FMC Medical Records Department, which is located on the first floor at 401 N. Ewing St. in Lancaster, Ohio. The hours are 7 a.m.-8:30 p.m.

How do I sign up for the medical records portal?

The staff members in the Medical Records department will be able to walk you through the process to sign up to the portal. Online self-enrollment is available if you are 18 and over. You will need your medical record number in order to successfully register online. Online self-enrollment is currently not available for caregivers or guardians.

Is Franklin Medical Center secure?

Yes. Frank lin Medical Center’s patient portal is a secure website. All information is encrypted, and you will be the only one who can view your health information. You will be required to setup a username and password when registering for access to the patient portal.

How to retrieve health information from Family Medical Center?

Retrieve and manage your health information online anywhere, anytime with your computer, tablet, mobile device or smart phone with Family Medical Center’s new patient portal, FollowMyHealth™. If you’re a patient of Family Medical Center, you can use FollowMyHealth to review your medical information online and communicate with your Family Medical Center office. This is a FREE service provided by Family Medical Center. In order to create your account, you’ll need an invitation code (usually the last four digits of your social security number). Please contact our office to enroll and receive your invitation email.

How long does it take to get a response from FollowMyHealth?

If you need urgent medical assistance, call 911. If you need a response from the office within 24 hours, please call the office. It may take two to three business days to receive a response to any FollowMyHealth communication with the office.

Can you see prescription refills?

You may see medications that have been prescribed by physicians other than a Family Medical Center physician, such as a specialist. Prescription refills can only be requested on medications that have been prescribed by your physician or one of their partners.

Is Follow My Health secure?

The Follow My Health website is secure and configured in such a way that hackers cannot access the information.

Can you call 911 for follow my health?

FollowMyHealth should never be used for urgent medical matters. If you need urgent medical assistance, call 911.

Can you check your progress with a care team?

You and your care team can check your progress, send secure messages and address any concerns or possible issues in between your monthly visits.
