Even though your hospital has a new name, all of your health information and medical records are still in the same place — your patient portal — but there are different portals, depending on when you became our patient.

This added security layer prompts you with a code sent to an email address to help ensure only you have access to your important information.

In compliance with new requirements of the Affordable Care Act, Florida Hospital Care Advantage has made available a Summary of Benefits and Coverage (SBC) for your plan. The SBC summarizes important information about your health coverage in a standardized format. Each SBC will display as a PDF, and you will need Adobe Reader to view it.
