When initiating an e-visit, the patient must accept the Terms and Conditions in this agreement, verify his/her information, and then complete an online form about his/her symptoms. This on-line form gives the health care provider the information needed to further assess treatment needs.

FirstHealth is also obligated to request legal or other supporting documentation showing to FirstHealth's satisfaction that, and the extent to which, a proxy is legally authorized to access a FirstHealth patient's information.
