My Health eChart Patient Portal gives you online access to your hospital-based electronic medical records on a convenient and secure site. Sign up is easy and lets you manage your health 24 hours a day, seven days a week from your computer or mobile device.

Is Family Health Services a non profit?

Family Health Services is a non-profit, Federally Qualified Health Center in Sandusky that provides primary care for adults, children and infants at their Hayes Avenue location, and primary care for adults at their E. Water Street location. Family Health Services accepts Medicare, Medicaid, and insurance from the majority of providers. For patients who have no insurance, services are offered on a sliding fee scale to assist people in affording quality health care.

For your convenience, our central billing office is staffed Monday through Friday from 8 a.m. to 4:30 p.m. You can call the office at 419-557-5530.

You can download, print and fill out some of your patient registration forms prior to your appointment to reduce wait times.

Advance Directives

Advance directives are legal written documents that indicate what an individual wants regarding care and treatment if he or she is not able to speak for himself or herself. There are three documents: a healthcare power of attorney, a living will and a do-not-resuscitate order. All must be signed by the competent patient and witnessed or notarized.
