For your convenience, we offer a number of ways to make payments on your account. If at any time you have difficulty paying by the methods outlined below, please feel free to call us and we will be happy to assist you.

Cryopreservation or Annual Storage Fee

For patients who are paying for cryostorage of embryos, eggs, and/or sperm, and/or monthly storage fees, please make payments through the Embryo Options Portal. Not currently enrolled with Embryo Options? Click here.

Statement Balances or Treatment Deposits

Double-check your patient account number and invoice amount to ensure accurate payment and quick verification.

By Phone

Simply call us at 847-729-2188 anytime during business hours and we can verify the payment due, set up payment arrangements, and take your credit card information over the phone. We accept Visa, MasterCard, American Express and Discover.

By Mail

To mail your payment to us, simply send us the top portion of your billing statement along with a personal check or your credit card information authorizing payment.

How to Obtain Medical Records

To obtain a copy of your medical records, please return a completed copy of the medical records authorization form. If you have not yet completed an authorization form to have medical records sent to you or another healthcare provider, see the link to download the authorization form.

Payment and Timeline for Medical Records

The normal turnaround time to receive copies of records is 30 days from the time payment is received. If you require the information in less time or on an emergent basis, you may contact our medical records department by calling between 7:30 AM – 4 PM CST, and we will do our best to accommodate your specific needs.
