Simply call us at 847-729-2188 anytime during business hours and we can verify the payment due, set up payment arrangements, and take your credit card information over the phone. We accept Visa, MasterCard, American Express and Discover.
To mail your payment to us, simply send us the top portion of your billing statement along with a personal check or your credit card information authorizing payment.
To obtain a copy of your medical records, please return a completed copy of the medical records authorization form. If you have not yet completed an authorization form to have medical records sent to you or another healthcare provider, see the link to download the authorization form.
The normal turnaround time to receive copies of records is 30 days from the time payment is received. If you require the information in less time or on an emergent basis, you may contact our medical records department by calling between 7:30 AM – 4 PM CST, and we will do our best to accommodate your specific needs.
