The goal of the family medicine specialty is to provide continual, comprehensive care to an individual. Family medicine advocates believe that seeing many different specialists can sometimes result in fragmented care.
Physicians who practice family medicine can treat patients regardless of age, gender, social status or health issue. Having a family doctor allows all members of the family, from infancy to an elderly age, to see the same doctor.
Have you had a recent visit with your UCHealth primary care provider? If so, it may be possible that your provider can sign the required forms without an actual appointment.
Many states, including Colorado, require sports physicals for kids once a year or before participating in any physical activity at a school, camp or community organization. Sports, school and camp physicals are available at select UCHealth primary care locations.
For Patients Who Are Minors: Due to federal and state confidentiality laws, patients who are between the ages of 14 and 17 are not required to disclose certain types of medical information to parents/legal guardians.
Parents/guardians may communicate with FPC providers and staff as you always have until your child reaches age 18.
For this reason, we find it necessary to ask our patients in this age group to complete a consent form naming a parent/guardian as their portal authorized representative. Without this consent from the minor patient, we will not be able to give parents/guardians access to the minor's portal. (Click the link below to access the form.)

The Patient Portal is not a substitute for seeing a physician in an office visit setting, but is merely a more convenient way for patients to access their own health information. The Patient Portal is also an easier way for them to stay current with their healthcare provider. For instance, if insurance or other information has changed, patients can simply login to their portal and verify their information.
The InteliChart Patient Portal technology streamlines the communication process between patients and their providers. Patients will be able to login to their patient portal and access information such as viewing their medication list and instructions and viewing the results from their latest laboratory test.
Other features of the InteliChart PHR include accessing current medication lists, verifying demographic or insurance information, viewing medical summaries, and leaving notes and other communications for the healthcare staff. Additionally, patients can receive alerts such as new medications and appointment reminders as text messages or emails, as they prefer.
