Our goals are to spend more time listening to and getting to know our patients, as we know that your your health and peace of mind are very important.

You can fill out an online form to request an activation code in the mail. This can take 5-7 business days.
You can use the activation code provided on your After Visit Summary (AVS) or billing statement.
If you want to sign up in person at any of our clinics, simply ask our staff to help you while you're there.

Caroline is a board certified Physician Assistant. She completed her undergraduate degree at NC State University and graduated from Duke University’s Physician Assistant program in 2017. Prior to joining the Family First team, she worked in internal medicine with a focus on geriatrics. She enjoys the variety and patient-provider relationship building that primary care offers; her particular interests include preventative healthcare, women’s health, and chronic disease management. Beyond her time in clinic, Caroline enjoys traveling, reading, and exploring the bountiful cultural scene of the Triangle.
Dr. Grace Tang did her undergraduate work at the University of Maryland Baltimore County. She studied medicine at Pennsylvania State College of Medicine and did her Family Medicine training at Franklin Square Hospital Center in Baltimore, MD. She grew up mostly in Canada and Maryland. She is board certified in Family Medicine. She practices the broad spectrum of family medicine. She particularly enjoys preventive healthcare and women’s health issues. She has practiced in the Wake Forest and North Raleigh area for over 10 years. She and her husband Brian have an awesome, but stubborn border terrier, Sandy. In her free time, Dr. Tang loves to travel, workout (sometimes), and craft.
