When to use the Ocala Family Medical Center medical records form?

Use this if you are a new patient to our practice or if you have not been seen in over 3 years. This form allows you to receive copies of your medical records or have your medical records sent to/from Ocala Family Medical Center from another Provider.

How do I make an appointment at FamilyCare specialists?

For more information about the primary care and walk-in services at FamilyCare Specialists or to request an appointment, call (865) 584-2146. On your first visit, you will be asked to fill out registration forms. You may choose to pre-register by downloading, printing and completing the forms below. Just bring them to your first visit.

What if I haven’t registered with the patient portal?

If you haven’t registered with our Patient Portal yet, please contact the office at 352-237-4133 so we can send you the registration email. The Patient Portal is a great tool to utilize as you can send messages to your Provider, request appointments, make prescription refill requests, pay your bill, and more!
