The Patient Portal is an online service that provides patients secure access to their health information. Any active patient over the age of 13 is eligible to register for and use the Patient Portal. If you are authorized, a family access account can be created that will allow you to access selected family members' health information. top

The Patient Portal gives you access to several different services. First, you can start your health journey by completing your Wellness Snapshot, which is a survey designed to give you a high-level look at potential health risks. In addition, you can schedule primary care appointments through the Patient Portal. Lastly, you can message our Care Coordination team regarding appointments, results, and patient records.
Using the button above to begin the registration process for the Patient Portal. You will enter your email address and create a password that will allow you to login into the portal in the future. After creating your login, you will enter your personal information. Once you’ve complete the registration steps, you will be taken to the portal dashboard. From there you can begin using the portal to complete your Wellness Snapshot or schedule a primary care appointment.
