This invitation to join expires after 90 days. If you do not have a scheduled appointment and wish to sign up for My FMC Portal, you need to go to the FMC Medical Records Department, which is located on the first floor at 401 N. Ewing St. in Lancaster, Ohio. The hours are 7 a.m.-8:30 p.m.
The staff members in the Medical Records department will be able to walk you through the process to sign up to the portal. Online self-enrollment is available if you are 18 and over. You will need your medical record number in order to successfully register online. Online self-enrollment is currently not available for caregivers or guardians.
